He is survived by: his wife Sharon Denice Brown Phillips; his sons, Dameon Michael Lev and Justin David Lev (Camille Dyan Broadus); his grandchildren, Tyler, Tanner, Dustin, Theo, Joseph, Reed and Alleen Lev; and his sister Maybelle O'Bannon Criner.
