He was predeceased by: his wife Nancy Catherine Usher; and his daughter Kathy Usher. He is survived by: his daughter Carolyn Carr; and his grandchildren, Jeremy Carr (Amber), Jessica Carr, Cathy Hall, Harvey Setliff and Angel Bertholf. He is also survived by numerous great grandchildren; and many other extended family and friends.
