He was predeceased by: his father David Lee; and his stepmother Judy Sullivan. He is survived by: his mother Carol Chambers Sullivan; his sons, Konnor Scott and Kason Jay; his wife Carla Blair; his brother John David Sullivan (Wendy); his sibling's children, Kelden, Shawn and Whitney Jo; and his cousins, James Fortenberry (Kelly) and Dede (Donald).

In lieu of flowers the family requests donations be made to Pet Adoptions of Cuero.
