He is survived by: his brother Todd (Nadine) of Farmington; his nephew Jason Anderson; his wife Stephanie; his son Roark (Tami); his granddaughters, River, Aspen, Harley and Bradlee; and his siblings, Ann Pyper (Ron) of Arizona, Ina Butterworth Jr. (Jay) of Cedar City, Rugene Jacobs of Idaho, Chris Axford (Craig) of Canada, Cilla Garrett of South Jordan, James Garrett (Cindy) of Fruit Heights and Kathy Garrett of South Jordan. He is also survived by his wife, son, parents, brothers, sisters and friends.
