She was predeceased by: her parents, Ray Hawkins and Montez Elliott Hawkins of Edgemont, Utah.; her brother Doug Hawkins; and her granddaughter Angela Worthen. She is survived by: her husband Richard Worthen (Dick); her children, Rick Worthen (Sandra), Ann King (Tran), John Worthen (Loraine), Chris Roten (Randy), Ray Worthen (Kim) and Troy Worthen (Jodi); her sister Dorthy Goulding; and her sister-in-law Audrey Worthen. She is also survived by 6 children, 24 grandchildren, 76 great grandchildren, and 11 great great grandchildren.
