He was predeceased by: his parents, Merlin Stringham Stevens and Georgia Whitaker Stevens of Lehi, Utah.; and his grandparents, Hazen, Hattie Stevens, Joseph and Minnie Whitaker.
He is survived by: his former wife Rosemary Jacobs; his daughter Angela (Angie); his wife Stella Stevens; his children, Mary Allen (Don Dee), Mindy Linn Brown (Darl), Hazen David Stevens (Angie), Coleman Jacob Stevens and Marcy Walker (Bryan); and his siblings, Phillip Stevens, Karen (Lamont) of Fillmore, Rosemary (Rod) of Beaver and Merlene (Glade) of Holladay, Utah.. He is also survived by 14 grandchildren and 12 great grandchildren.
