It is with great sadness that we announce the death of Jay Anderson Haslam (Logan, Utah), who passed away on June 15, 2018, at the age of 83, leaving to mourn family and friends. He is survived by: his spouse Connie Petersen; his children, Deborah Haslam (Mike) and Lyle Haslam (Julie); and his grandchildren, Jeff, Joey, Austin and Lindsay.
