He was predeceased by: his parents, Frank Hazen Sr. and Rose Dean; his wife Melba; and his siblings, Ruth Jensen, Edna Brown, Marjorie Hazen and Jack Hazen.
He is survived by: his brother Bob Hazen; and his children, Christine Webb (Mike), Craig Hazen (Justine), Michelle Wilde (Dean) and Denise McCashland (Kent). He is also survived by 12 grandchildren and 20 great-grandchildren.
