He was predeceased by: his parents, Raymond William and Mary Jane McKinley McIntosh; and his siblings, John, Robert, Ruth and Mary.
He is survived by: his spouse Loretta Buck; and his children, Sherrie Lynn McIntosh Malcolm (Ronald), Rodney Lee McIntosh (Lisa) and Nicolas Ray McIntosh (Karen). He is also survived by his wife, children, grandchildren, great-grandchildren, and many other family and friends.
In lieu of flowers, the family asks that a donation be made to the Ogden Rescue Mission.
