He was predeceased by: his parents, George Frank and Sylvia Frank; his wife Jerri Frank; his daughter Michelle Frank; his stepfather Ralph Balzy; his parents-in-law, Mark and Leah Butler; and his siblings, Reed, George, Floyd, Johnny, Hyrum Frank, Sylvia Lee, Sheila Frank and Ruby Frank.
He is survived by: his former spouse Junior High; and his children, Allyson Nielsen (Ron), Shane Frank (Janet) and Heather Slaymaker (Cory). He is also survived by 16 grandchildren, and 9 great-grandchildren.
