He is survived by: his daughters, Brenda Archuleta (Craig) of Brigham City, Caryn Ryan (Kelly) of Clearfield and Amy Bair (Bryan) of Clinton, Utah.; his brother Leo Cook (Marlene) of Syracuse; and his sister-in-law Deanna Cook of Syracuse, Utah.. He is also survived by companion as his daughters and granddaughters.
