He was predeceased by: his parents, Joseph Richard Forrest, III and Jane Wiltermood Forrest. He is survived by: his children, Kayla Forrest Harrison (Cody) and Justin Forrest (Stephanie); his siblings, Jean Forrest, Dori Thurman (Josh), Debbie Peterson and John Forrest; his niece Jazmine Powell; and his nephews, K. J. Powell, Joshua and Jacob Thurman.
The family suggests that memorial donations be made to St. Jude Children's Research Hospital or Courtland Volunteer Fire Department.
