He was predeceased by : his father John Earl Hazelwood. He is survived by : his mother Katherine "Boots" Hazelwood; his sister Chelsey H. Malone (Michael); his nephew Coy Malone; and his grandmother Marie Martin. He is also survived by aunts, uncles, and cousins.
