He was predeceased by: his mother Georgia Martin (Flowers); and his grandparents, George, Blanch, Nonnie, Flowers, Cecil Smith and Vera Smith.
He is survived by: his daughters, Aero Konsavage (Smith) and Eden Oliver (Smith); his wife Dena Conklin; his grandchildren, John of Nicole Victor, Taylor and Caleb; his father Curtis Smith; his stepmother Sandra; his sisters, Brandee Perkins and Shannon Langland; and his stepfather Gerry Martin.
