He was predeceased by: his aunt Barbara Baumgart. He is survived by: his parents, Chris Sullivan and Carol Sullivan (Quella); his siblings, Michal (Abbey), Jaclyn (Luke), Ashley Zajkowski (Drew), Amber and Paige; his niece Everly; and his grandparents, Eugene, Sally Quella, Richard and Elaine Sullivan. He is also survived by 16 aunts and uncles, 30 cousins.
In lieu of flowers, monetary gifts can be sent to Thomas Sullivan and will be donated to a future charitable memorial in his name.
