He was predeceased by: his parents, Frank Ptacek and Sara Ptacek (Fox); his former wife Donna; and his siblings, Theodore, Franklin, David Ptacek, Betty Foust and Helen Erickson.
He is survived by: his wife Loreen Swaenepoel; his children, Randy Ptacek Jr. of Ladysmith, WI, Stacey Ptacek (Rob) of Menomonie, WI, Mindy Ptacek of Gilman and Mitch Ptacek (Jacquie) of Seattle, WA; his grandchildren, Caleb, Kelsie Macke, Taylor, Bristol Ptacek, Elisabeth Olsen, Anelise Larson and Oliver Ptacek; his great grandchild Greyson; and his siblings, Violet Zuber, Carol Draus, Linda Leach (John), Melody Fox (Jay), Gary Ptacek (Vicki) and Terry Ptacek (Vicki). He is also survived by many nieces nephews and friends.
