He was predeceased by: his wife DoDo; his parents-in-law, Edward and Marcella Newtols; his granddaughter Brittany Cayemberg; his siblings, Agnes Schuh (Leo), Donald Kozicke (Judy) and Evelyn Hansen; his brother-in-law Cletus Newtols (Rose); his nephews, Dennis Kozicke and Darrell Kozicke; and his goddaughter AnnMarie Vogels.

He is survived by: his children, Christopher Kozicki (Jane) and Michelle Lane (David); his grandchildren, Dr. Andrew Kozicki (Chelsea), Katelyn Kozicki (Brad), Alison Cayemberg (Josh) and Deanna Krause (Troy); his great grandchildren, Jackson, Wyatt and Jemma; his brother Gary Kozicke (Christine); and his goddaughter Sarah Knutson. He is also survived by numerous nieces and nephews.

In lieu of flowers, tax deductible donations can be made to Dale's favorite charity, The Brittany Cayemberg Foundation, in memory of his granddaughter. Donations will allow high school seniors to receive college scholarships.
