He was predeceased by: his parents, William Polasek and Eleanor Polasek (Bartkoviak); his former wife Helen Langfeldt; his wife Jane Speicher; his siblings, Earl Basil Polasek (Bernice) and Dolores Wright (William); and his nephew Brian Polasek.

He is survived by: his sister JoAnn Gregory; and his sibling's children, Raymond Wright (Christy), Diane Dictus (Steve), Robert Wright, Kathleen Wright (Jim Sivanich), Barbara Kovacs (Steve), Janet Janssen (Doug), Bruce Polasek (Kimberly), Philip Polasek (Leslie), Ann Polasek, Mike Gregory (Pam), Colette Gregory, Keith Gregory and Todd Gregory (Kim). He is also survived by many great-nieces and nephews, great-great-nieces and nephews, other relatives, and friends.
