He was predeceased by : his father David John; his sister Carrisa Lee Smith; his grandfathers, Hubert Smith, Virgil Bieger and James Hogan; his father-in-law Rex Madesen; and his sister-in-law Faye Madesen.
He is survived by : his former life partner Jennifer Lynn Madesen; his wife Jennifer Smith of Juda, Wisconsin; his son Caleb Smith of Juda, Wisconsin; his mother Mary Jo; his stepfather Jerry Hegarty of Oelwein; his stepbrothers, Jeremy Hegarty (Suzanne) of Adel and Tim Hegarty (Jennifer); his grandmothers, Loretta Smith of Oelwein and Darlene Hogan of Oelwein; his mother-in-law Lena Madesen of Indianola; and his siblings-in-law, Brenda Graham (Kevin) of Ackworth, DeAnne Gillette (Ron) of Norwalk, Maxine Irish (Scott) of Indianola, Robert Madesen (Pam) of Indianola and Steve Madesen (Penny) of Albuquerque. He is also survived by several nieces and nephews.
