He is survived by: his wife Cindy Hazen; his son Dustin Fairbanks; his brother Jeff Fairbanks; his stepchildren, Kelly Leisner and Richard Gabriel (Jayne); and his grandchildren, Dameon Hill, Jackson Leisner, Aria Leisner, AJ Gabriel and Joyce Gabriel. He is also survived by several aunts, uncles and cousins.
