She was predeceased by : her parents, Walter Theusch and Regina Theusch; her husband Roger Jacak; her siblings, Walter Theusch (Sonny), Robert Theusch, Francis Theusch, James Theusch, Richard Theusch and Marilyn Theusch; and her brothers-in-law, Ervin Beine, Sylvester Jacak (Betty), Lambert Jacak (Irene) and Rollie Jacak (Marian).
She is survived by : her children, David Jacak of Campbellsport, Mary Jacak-Abraham of Campbellsport and Roger "Butch" Jacak Jr. of Campbellsport; her grandchildren, Davey, Jarod, Heather, Danyl, Michael and Kathy; her great grandchildren, Hailey, Andrew, Alexis, Addison, Aubrey, Keaton, Zach, Mikayla, Madison and Taylor; her siblings, Lorraine Beine, Rosemary Guth (Robert), Elaine Wieberdink (Dennis), Darlene Walls and Donald Theusch (Dianne); and her sisters-in-law, Caroline Theusch and Carol Theusch. She is also survived by other relatives and friends.
