He was predeceased by : his father Terrence D. Kolb; and his brother Freddie Smith. He is survived by : his mother Beverly A. Kolb (Corey) of Reedsburg; and his siblings, Timothy Kolb of Reedsburg, Jill Graham of Reedsburg, Kim Farmer of Joliet, IL, Barbara Watson of Statesville, NC and Kori Smith of Reedsburg. He is also survived by several nieces, nephews, along with other relatives and friends.
