She was predeceased by: her husband L. Wayne Jack; her brothers-in-law, Alvin R., Tharon Jack and Paul McQuain; and her great grandchildren, Jack Wilfong and Bella Reece.

She is survived by: her children, Michael Jack (Donna) of Virginia and Sue Reece (Mitch) of Elkins; her sisters-in-law, Judy Jack and Sharon McQuain of Elkins; her grandchildren, Janae Daigle (Justin Frye), Samantha Cutshaw (Otis), Shannon Wilfong (Patrick) of Montrose, Rebecca Derring (Scott) of Elkins, Jaimie Roberts (Butch), Rachel (Tori Barlow) of Elkins, Nick Reece of Huntington, WV, Magen Cheatham (Dell) and Matthew Jack; and her great grandchildren, Kiersten, Tyler, Chayse, Shayne, Savannah, Mason, Donovan, Aiden, Annaleigh, Audrey, Ashlyn, Jordan, Chloe, Madison, Adriana, Raegan, Camden, Sydnee, Lukas, Coltyn, Parker and Elizabeth.

Masks and social distancing will be required for anyone attending the services.

Memorial contributions may be made to the Charity of Choice in memory of Mrs. Jack.
