He was predeceased by: his parents, Harold Forman and Sara Smith (Rodgers); and his sister-in-law Patty Forman. He is survived by: his brother Mike Forman of Leesburg, FL; his daughters, Tracy Pavalok (Randy) of Grant Town, WV and Kerry Kidder (Joshua) of Fairmont, WV; his grandsons, Dylan, Kaden Pavalok and Miles Kidder; his niece Kristy Kelley (Terry); and his nephew David Forman.
