He was predeceased by: his grandfather Kenneth J. Mallory. He is survived by: his sisters, Jessica Graham (Matthew) of Franklin and Heather Jefferson (Jonathan) of Huntington; and his grandparents, Greta Mallory of South Charleston, William Claude and Mildred Casto of Weston.

In lieu of flowers donations may be made in Jeremy's memory to Putnam County Animal Shelter at 12908 Charleston Road, Red House WV 25168.
