He is survived by: his former wife Caroline Kay Miller; his sons, Jim Hayworth (Kim) of Belle Fourche and Gary Hayworth (Carrie) of Belle Fourche; his grandchildren, Michael, Brandon, Kyrstin, Dante, Dierks and Mary Jane; and his siblings, Wanda Hayworth, Sarah Roth, Susan Hayworth, Roger Hayworth, Tim Hayworth, Verlin Hayworth (Marcia) and Cy Hayworth (Rachel). He is also survived by numerous nieces and nephews.
