She was predeceased by : her parents, Stanley Polczak and Anna Polczak; her husband Charles Ballard; her son Michael Ballard; her granddaughter Nikki Ballard; and her siblings, Raymond Polczak, Ted Polczak, Virginia Doviak, Stella Delia and Irene Petta.

She is survived by : her children, Patrick Ballard (Janice) of Birmingham, AL, Kevin Ballard (Peggy) of Quincy, IL, Jenny Connelly (Kevin) of Wellsboro, PA, Anna Martinez (John) of Wichita Falls, TX, Lucy Maiorano (Rich) of Prescott, AZ, Rebecca Spray (Don) of Mansfield, TX, Rachel Diserens (Daniel) of College Station, TX and Jeannie Ballard West of Dallas, TX.; and her sister Lucy Emory of Mexico, NY.. She is also survived by 26 grandchildren and 40 great-grandchildren.
