He is survived by: his wife Julie; his daughters, Chelsea (Jon), Breanna (Russell) and Margaret; his grandsons, Dylan, Nolan, Tiernan and Dominik; his siblings, Curtis (D'Etta), Melvin (Audrey), Verna (Tyrone) and Anthony Polk (Rose); and his sister-in-law Carlotta. He is also survived by many nieces and nephews.
