He was predeceased by: his father Girard. He is survived by: his sons, Matthew and Harrison Costello; his mother Gayle Costello; his former wife April Hanley; his brother Mark; his sister-in-law Jacki; his niece Olivia; his nephews, Jack and Liam; and his brother Bill.
In lieu of flowers, the family kindly requests that donations be made to the Kenai Christian Church, whose support was a tremendous comfort to Dan and his family during his illness.
