He was predeceased by: his wife Joyce Elaine (Boult); his parents, Paul Eizenga and Hendericken Hattie Van Eizenga (Holtgeerts); and his siblings, Gerald (Mildred), John (Yvonne), Jane Tunstra (Sid), Paul "Dutch" (Charlene), Ray (Betty) and Eileen Ruiter (Gerrit).

He is survived by: his children, Jerry Van Eizenga (JoAnn) and Pamela Graves (Howard); his grandchildren, James (Joanna), Shawn (Megan), John and Shelby Van Eizenga; and his great grandchildren, Marley, Zuri and Kenji Van Eizenga.
