He is survived by: his children, Ted Evans (Shelli) and Tracy Kozack; his brother Jimmy Polka (Joyce); his grandchildren, Evan Innerarity (Jessica Bolan), Myka Kerr (Colin), Tara Schneider (Jason), Torrin Mallory (Jennifer), Dexianna Evans and Ben Dickson; and his great grandchildren, Aydon Kerr, Joylin Kerr, McKenzie Kerr, Scarlet Schneider, Halle Schneider, Pierce Schneider, Tevin Mallory, Josiah Rector and Rosely Rector.

In lieu of flowers, the family has requested that donations be made to American Cancer Society, or your favorite charity.
