He was predeceased by : his parents, Hershel Cook and Wynell Cook; and his siblings, Donald Cook and Barbra Sanders. He is survived by : his wife Terri Cook of Boligee, AL; his sons, Jeremy Cook of Berry, AL and Christopher Swindle of Fayette, AL; and his brother Terry Cook of Tuscaloosa, AL. He is also survived by three step children he loved.
