He is survived by: his wife Jane White Blevins; his sons, Matthew Blevins (Katy), Steve Page (Natalie) and Todd Page; his grandchildren, Will Blevins, Katy Blevins, Pasha Page, Sophie Page, Claudia Sparks and Will Sparks; and his sister Marcie Keith. He is also survived by several nieces, nephews.

In lieu of flowers, memorials may be made to St. Jude and Hospice of the Valley.
