He is survived by: his wife Ann Forehand; his children, Teresa Wells (Keith), Eddie Forehand, Shawn Lloyd (Pam) and Amanda Anderson; his grandchildren, Blake Dorminey, Jarred Dorminey, Lauren Dorminey, Kayla Lloyd, Tanner Love, Tommy Love, Keith Jr. Wells, Erica Wells and Keegan Wells; his great grandchildren, Kase, Carter, Rilynn, Skylar, Emma Lee and Ariel; and his siblings, Betty Wood and Eugene Forehand (Jean). He is also survived by several nieces and nephews.
