He was predeceased by: his father James Jefferson Cook; his significant other Polly A (Cook); and his siblings, Myrl Gambill (Bill), Ruby Parker (Clyde), Earl Lewis (Mary) and Lester Cook (Vivian).

He is survived by: his children, Sheryl Cook and Jeff Cook (Laura); his granddaughter Jerica Cook Pinson (Garrett); his great grandchild Noah Pinson; and his niece Cathy Tolar.
