He was predeceased by: his parents, Leamon Cook and Mildred Tindell Cook; and his children, Kristie Marie Cook and Timothy Cook. He is survived by: his wife Jackie Cook; his daughter Karen Kornegay; his daughter-in-law Lea Ann Cook; his grandchildren, Chase Kornegay, Carson Kornegay, Carley Cook and Cameron Cook; and his siblings, James Cook (Anna), Linda Parker (Tom), Martha Smith (Ronnie) and Debbie Cook. He is also survived by several nieces, nephews, and other extended family and friends.
