He is survived by: his sons, John "Johnny" Velazquez of Merrillville, Indiana, Sergio Velazquez of Mesa, Arizona and Maxwell Velazquez of Mesa, Arizona; his siblings, Ana Kenefick of Gulf Stream, FL and Dino Velazquez (Ann); his grandchildren, Emma Velazquez and Johnny Velazquez; and his siblings' children, Laura Suckey (Troy), Matt Milanowski (Dana) and Kristin Velazquez Ross (John).
