He is survived by: his parents, Donald Wayne White and Nelda Jean White; his wife Tammy Mahaffey; his children, Patrick Revels (Adrianna), Austin Mahaffey (Maryann), Christine Duncan (Kenneth), Jordan Konrad (Matt) and Kristen King (Chase); his grandchildren, Carolyn Shank, Laney Revels, Joclyn Shank, Charlie Shank, Jasper Shank, Rylie Gasper and Paxton; his sister Rhonda Tolbert; his nephews, Shane Barnhill (Toshia), Brandon Barnhill (Nicole) and Matthew King (Whitney); and his niece Jessica Snyder (Cody).
