He is survived by: his wife Debbie "Deb" Cook; his children, Jonathan Cook (Leslie), Leslie Cook, Becca Cook, Tiffany Hughes (Jerry) and Traci Bennett; and his grandchildren, Seth Cook, Leah Chahrour (Ali), Chad Hughes (Kayla), Danielle Hughes (Will), Aden Cook, Shelbi Bennett and Katelyn Bennett.
