He is survived by: his mother Carolyn Cook (Morley Johnson); his grandfather Jack Thomas Cook Sr.; his brother Don Cook; his sister-in-law Mary Beth Cook; and his nephews, Hunter, Best Friend and Nickie Wetherell.
