He is survived by: his daughter Andrielynne Chamberlain; his parents, Karen Prescott (Thomas) and Lawrence Chamberlain; and his siblings, Leigh Adams (Dylan), Laura Chamberlain, Melinda Bradley (Joel), John Ryan Fields (Jamie), Jordan Renee Hoch (Evan), David Prescott (Macey) and Kelli Lynne Prescott. He is also survived by several aunts, uncles, nieces, nephews and cousins.
