He is survived by : his wife Mary Helen Bowen Forrest; his children, Philip Ryder Forrest, IV, Katherine Demeranville Forrest, Matthew Todd Forrest, Ellen Theresa Forrest and Mary "Mimi" Elizabeth Forrest Slothower (Matthew Slothower); his grandchildren, Anna Hebert Marie and Gustave Anthony Marie, IV; his great grandchildren, Savannah Rose, Declan Paul and Hudson Anthony; and his children, Julia Elizabeth Johnson, William David Johnson, Jr. and Susanne Bowen Johnson. He is also survived by great grandchildren.
