He was predeceased by: his parents, R. L. Shook and Shelva Jean Shook (Jones); and his son Randall Scott Shook. He is survived by: his wife Blanche Annette Shook (Thomas); his children, Gregory Todd Shook (Erica) and Pamela Alisha Shook (Carrie); and his siblings, Reginald Lynn Shook (Susan), Connie Shook McGee (Ollie) and Mickey Garth Shook (Pam). He is also survived by 8 Grandchildren.
In-lieu of flowers, the family requests that donations be made to the Leukemia & Lymphoma Society (LLS) or the Alzheimer's Association.
