He was predeceased by: his parents, Glenn Smith and Lucille Smith; his former wife Lorraine M. Smith; his son Stephen Thomas; and his sister Janice Palm.

He is survived by: his wife Lori; his children, Dr. J. D. Smith (Anna) of Florence and Dr. Stuart Smith (Ingrid Van Duyne) of Madison, Wisconsin.; and his grandchildren, Jack (University), Scott, Mark, Tricia and Auburn University. He is also survived by his wife, children, and grandchildren.
