He was predeceased by: his parents, Alvin Samples, Sr. and Peggy Samples; and his siblings, Bonnie Murray, Joanne Smith, Virginia Murray, Charles Sides and Ronald Samples.

He is survived by: his wife Carol Ann Samples; his children, Craig Samples (Krista) and Ashley King (Jerry); his grandchildren, Hunter Samples, Tripp King and Chaney King; and his siblings, Ray Samples (Jackie), Peggy Galbaugh and Harold Samples (Linda). He is also survived by nieces, nephews, extended friends and family.
