He is survived by: his wife Deborah Sample; his son Joseph "Nicholas" Sample; his grandson Jacob Daniel Sample (Hannah); his great grandchild Savannah Jean; and his siblings, Robert David Sample (Gloria), Michael Sample (JoAnn) and Paula Kaye Sample Eubanks (Ron).
