He is survived by: his wife Shelia Cook; his children, Mindy Stewart and Clint Cook; his grandchildren, Caroline Pearl Stewart, Parker Alan Cook and Joseph Eli Olive; his mother Sara Ruth Cook; and his siblings, Seconda Cook and Rhonda Lou Witcher.
