He was predeceased by: his parents, Nick I. Welk and Lanell Summers Welk; his grandparents, Jesse, Lottie Summers, August and Doris Welk; and his nephew Eric Welk.
He is survived by: his wife Cynthia Deshazo Welk; his children, Sabrina Bonner (Shelley), Danny Hicks, Donny Hicks and Waylon Welk (Kayla); and his siblings, Donna Elliott, Larry Welk (Janet) and Frank Welk (Jennifer). He is also survived by several nieces, nephews, aunts, and uncles, and other extended family.
