He was predeceased by: his father John David; his brother David Wayne Lewis; and his nephew John Wayne Lewis. He is survived by: his sisters, Myrtle Marie Underwood, Nita Faye Denson (Mitch) and Judy Darleen Ingram (Brian); and his sister-in-law Vickie Ann Lewis. He is also survived by six nephews, eight nieces, and numerous great-nieces and nephews.
