He was predeceased by: his daughter Margie Cook Rice; his parents, Oscar Ezra and Nancy Cook; and his siblings, Grady, Wentfline, Polly, Jean, Jackie and Louise.
He is survived by: his wife Connie Cook; his children, Billy Cook, Nancy Bysko (Richard) and Jimmy Cook (Rachel); his grandchildren, Chad Bysko (Danielle), Samantha Clanton (Justin), Cassidy Butler (Quinton), Loyal Dean (Victoria) and Hannah Cook; his great grandchildren, Clare Cullum, Kash Butler and Wyley Bysko; and his siblings, Inez Yealock, Fayerene Morgan, Bobby Cook (Hazel) and Loyd Cook (Sue). He is also survived by nieces and nephews.
